Connecting you to community resources

Because our staff also includes medical social workers, Home Nursing With Heart is able to maintain relationships with other community organizations who provide additional support services that can help with housekeeping and daily living activities, medication and household financial assistance, counseling and psychological support, depression and more. As a result, we’re able to help patients and families resolve social, financial and psychological challenges related to their health condition.

Helping you to know your rights

As patients, Home Nursing With Heart feels you should know your rights when it comes to home health services.

  • Federal and state legislation guarantee your right to choose the home health care provider that you want in your home.
  • Hospital discharge plans must include an evaluation of a patient’s possible need for medically necessary home health care

In short, you have the right to choose the type of care that is best for you. You also have the right to choose the provider best suited to your medical, situational and personal needs.
